VOLUNTEER STATUS I understand and acknowledge that I am participating in activities with Psalm 72 voluntarily and without expectation of wages, compensation, employee benefits, or other remuneration unless Psalm 72 has expressly agreed otherwise in writing.
I understand that my volunteer service does not create an employer-employee relationship between me and Psalm 72.
I understand that I am free to discontinue my volunteer service at any time and that Psalm 72 may limit or discontinue my participation as a volunteer at its discretion.
ACKNOWLEDGMENT AND ASSUMPTION OF RISK I understand that volunteering with Psalm 72 may involve activities that carry inherent and other risks of injury, illness, property damage, disability, or death.
Depending upon my volunteer assignment, these activities may include, but are not limited to, construction, renovation, demolition, painting, cleaning, landscaping, lifting and carrying materials, moving furniture or equipment, climbing stairs or ladders, using hand tools or power tools, working around construction materials, electrical equipment, vehicles, machinery, dust, debris, chemicals, uneven surfaces, and other potentially hazardous conditions.
Volunteer activities may also include ministry events, outreach activities, food preparation or service, transportation, travel, and activities at locations not owned or controlled by Psalm 72.
I understand that these activities may expose me to risks including, but not limited to, falls, cuts, burns, strains, sprains, broken bones, falling objects, electrical shock, exposure to dust or chemicals, heat or cold, vehicle accidents, illness, serious bodily injury, permanent disability, and death.
I knowingly and voluntarily accept and assume the risks associated with my participation in Psalm 72 volunteer activities.
I understand that I may decline to perform a task that I reasonably believe I am not qualified, physically capable, properly equipped, or adequately trained to perform.
I HAVE READ AND VOLUNTARILY ASSUME THESE RISKS
RELEASE AND WAIVER OF LIABILITY To the fullest extent permitted by Pennsylvania law, I voluntarily release, waive, and discharge Psalm 72 and its directors, officers, employees, agents, representatives, and volunteers from claims or causes of action for personal injury, illness, death, or damage to or loss of property arising from my participation in Psalm 72 volunteer activities, including claims arising from the ordinary negligence of the released parties.
I understand that by signing this agreement I may be giving up important legal rights, including the right to bring certain claims against Psalm 72.
Nothing in this agreement is intended to release or waive liability that cannot legally be released or waived under applicable law.
I UNDERSTAND THAT THIS AGREEMENT INCLUDES A RELEASE AND WAIVER OF CERTAIN LEGAL CLAIMS
EMERGENCY MEDICAL TREATMENT If I become injured or ill while participating in a Psalm 72 activity and I am unable to make medical decisions for myself, I authorize Psalm 72 representatives to obtain or arrange reasonable emergency medical assistance on my behalf.
I understand that Psalm 72 cannot guarantee the availability or outcome of medical treatment.
Except to the extent otherwise required by law or covered by applicable insurance, I understand that I am responsible for medical, ambulance, hospital, or other healthcare expenses incurred as a result of my participation.
I AUTHORIZE EMERGENCY MEDICAL ASSISTANCE WHEN REASONABLY NECESSARY
INSURANCE ACKNOWLEDGMENT I understand that I should not assume that Psalm 72 provides me with health, medical, accident, disability, workers’ compensation, automobile, or other insurance coverage because of my status as a volunteer.
I understand that I am responsible for determining what personal insurance coverage I need or maintain in connection with my volunteer activities.
I UNDERSTAND AND ACKNOWLEDGE THE ABOVE
PHYSICAL ABILITY AND PERSONAL RESPONSIBILITY I represent that, to the best of my knowledge, I am physically and mentally capable of safely participating in the volunteer activities I choose to perform.
I agree not to participate in an activity that I reasonably believe exceeds my abilities or presents an unreasonable risk to my health or safety.
I will inform the appropriate Psalm 72 leader if I require a reasonable accommodation or if a condition arises that prevents me from safely performing an assigned task.
I understand that I am responsible for using reasonable judgment concerning my own safety.
SAFETY RULES AND INSTRUCTIONS I agree to follow reasonable safety policies, instructions, posted warnings, and directions provided by Psalm 72 or the person supervising the volunteer activity.
I agree to use required personal protective equipment and safety devices when instructed to do so.
I will not operate equipment, machinery, vehicles, power tools, or perform specialized work unless I have been authorized to do so and reasonably believe I have the necessary knowledge, training, or supervision.
I agree to promptly report injuries, accidents, damaged equipment, unsafe conditions, and other safety concerns to a Psalm 72 leader.
I will not participate in volunteer activities while impaired by alcohol, illegal drugs, or any other substance that makes me unable to perform the activity safely.
I understand that Psalm 72 may immediately stop me from performing an activity that it believes is unsafe.

TRANSPORTATION AND TRAVEL I understand that some volunteer activities may involve transportation or travel in vehicles owned, rented, borrowed, or operated by Psalm 72, another volunteer, or a third party.
I understand that motor vehicle travel involves risks including accidents, injury, disability, property damage, and death, and I voluntarily assume the ordinary risks associated with such transportation.
If I operate a vehicle in connection with Psalm 72 activities, I represent that I possess a valid driver’s license appropriate for the vehicle being operated and will comply with applicable traffic and safety laws.
I understand that authorization to drive on behalf of Psalm 72 may be subject to additional requirements, including proof of insurance and approval by Psalm 72.
RESPONSIBILITY FOR MY CONDUCT I agree to act responsibly and to follow applicable laws, Psalm 72 policies, safety requirements, and reasonable instructions while volunteering.
I understand that I may be held responsible, to the extent permitted by law, for injury, damage, or loss caused by my intentional misconduct, unlawful conduct, or reckless disregard of established safety requirements.
GOVERNING LAW I agree that this agreement will be interpreted under the laws of the Commonwealth of Pennsylvania, without regard to its conflict-of-law principles.
To the extent permitted by applicable law, any legal proceeding concerning this agreement will be brought in a court of competent jurisdiction in the Commonwealth of Pennsylvania.
SEVERABILITY If any provision of this agreement is determined to be invalid, illegal, or unenforceable, I agree that the remaining provisions will continue in full force and effect to the fullest extent permitted by law.
ENTIRE AGREEMENT AND UNDERSTANDING I acknowledge that I have had the opportunity to carefully read this agreement before signing it.
I understand that this document contains important provisions concerning assumption of risk, release of liability, and my legal rights.
I acknowledge that I am signing this agreement voluntarily and have had the opportunity to ask questions or seek independent advice before signing.
I understand that no oral statement or representation changes the terms of this agreement unless Psalm 72 agrees to the change in writing.
